Introduction: Cesarean sections are considered a true 'epidemic' due to the increase in rates over the years. Objective: To analyze the trend of cesarean sections in Brazilian capitals from 2009 to 2018. Methods: Ecological study of time series. The proportion of cesarean delivery in the state capitals was calculated from 2009 to 2018, using data from the Information System on Live Births (SINASC). To evaluate the trend of the proportion of cesarean sections, the Prais-Winsten regression model was considered, analyzed in STATA 14.0. Results: Black/brown and other women, aged 20 to 34 years and with 8 to 11 years of schooling, prevailed. The highest proportion of cesarean sections occurred in Goiânia, in 2012 (75.3%) and the lowest in Macapá in 2011 (33.6%). The highest proportions of cesarean section occurred in the capitals of the Southeast, South and Midwest. Conclusion: The findings showed that all Brazilian capitals have a proportion of cesarean sections above that recommended by the World Health Organization, and in some, there is an increasing trend, reinforcing the need for more effective public policies
