Aortic aneurysm diagnosed during pregnancy

Introduction: Abdominal aortic aneurysm (AAA) is characterized as a pathological focal dilation of the abdominal aorta, presenting a diameter greater than 50% of the normal vessel diameter. Symptoms usually manifest insidiously, and its discovery is often incidental, although it is associated with a reserved prognosis during pregnancy. Case report: This article presents the case report of a 42-year-old hypertensive pregnant woman, at 22 weeks and 3 days of gestation, admitted to the obstetric ward of a tertiary hospital due to a hypertensive spike. During obstetric ultrasound, a fusiform aneurysm was incidentally diagnosed, located below the emergence of the renal arteries and above the bifurcation of the iliac arteries, without thrombi, measuring 7.4 x 7.1 cm. An evaluation by a multidisciplinary team was conducted to determine the ideal timing for pregnancy interruption and the type of aneurysm treatment, aiming for the best maternal-fetal outcome, both successfully achieved. Conclusion: This report highlights the challenge of managing an AAA during pregnancy, the ideal gestational age for pregnancy interruption, and the complexity of the interventions required for aneurysm treatment.
